1. The Sky10 Lucky Draw (“Lucky Draw”) is open to all patrons of Sky Pilates Pte Ltd and Sky Physiotherapy Pte Ltd (collectively, the “Companies”) who are at least 18 years old, except the following categories of people: staff, sponsors, auditors and printers directly involved in this lucky draw.

2. Registration of Lucky Draw chances implies automatic acceptance of Terms and Conditions of the Lucky Draw.

3. A minimum spend of $500.00 on the Companies’ packages on a single same receipt is required to qualify for the Lucky Draw.

4. Receipts are not transferrable or reusable.

5. Limited to 1 Lucky Draw chance with every $500 spend / charge on packages from the Companies.

6. All entries received before Mon 31 Aug 2015 by 6:00pm will qualify for the Lucky Draw.

7. The Lucky Draw will be conducted on Tue 1 Sep 2015 at 583 Orchard Road #09-01 Forum (Office Tower) Singapore 238884.

8. One (1) winner will be drawn for each prize (in the prize list) in the Lucky Draw.

9. Winners will be contacted through the email provided to the Companies and must satisfy a verification process before being declared the winner. If a participant cannot be contacted or fails to respond by 6:00pm on 15 Sep 2015, this participant will be disqualified and another winner will be selected.

10. The Companies’ Management’s decision is final and irrevocable, and no correspondence will be entertained.

11. The award of the prizes are subject to certain terms and conditions and the winners must agree to such terms and conditions in writing in order to receive the prize. The Companies reserve the right to declare no winner for any draw if it is unable to contact or verify participants after repeated attempts or for any other reason whatsoever.

12. Prizes are not refundable or exchangeable for cash or in kind. The Management reserves the right to replace prizes with items of similar value.

13. Each participant’s personal data will be collected, used, disclosed and/or processed by the Companies for the following purposes:

(a) For the Companies or its designated representatives or business partners to provide goods and services to you or parties designated by you and matters ancillary thereto.

(b) To verify and process payment when you purchase goods and services from Companies.

(c) For verification and record of your personal particulars including comparing it with information from other sources and using the information to communicate with you.

(d) For research and analysis, including surveys and polls.

(e) To send you notices, information, promotions and updates including marketing and advertising materials in relation to the Companies’ goods and services and those of third party organizations selected by the Companies.

(f) To comply with any request from any third party or any order of court or directive from authorities investigating any alleged offence or misconduct or for the purposes of taking legal action against any you.

14. By participating in this contest, each participant consents to the Companies collecting, using, disclosing and/or processing your personal data for the purposes as described in Clause 13 above.

15. We take our responsibility to protect client information very seriously and will not sell or disclose our clients’ personal information to third parties without consent.
